A chevrolet dealer that has a GOOD body shop should have the code. Talk to the oldest/grayest looking person in the parts/body department and ask them about this. A good dealership will attempt to repair rather than just try to sell you a new wheel.
Your best bet may be to contact a wheel repair shop.
Google for these near your area-under "wheel repair" catagory. They repair usually aluminum or magnesium wheels-painted or polished. They would have the code or could quote you a price to get your wheel repaired professionally. They would be able to remove the curb rash also. They can actually machine the damaged wheel if necessary.
Or a Discount/American Tire shop may know a good and reasonable wheel repair shop in your area. They would see lots of damaged wheels in their normal work day .
