Don't know what year your car is but I just redid an '86 headliner.
Simply peel the old headliner off very carefully. I started at the one corner and it came off very easily. Try not to rip the old headliner because it can be used as a pattern. Didn't have to remove any mouldings. Cleaned all the loose padding from the roof (this stuff tends to dry rot over time and looses it adhereing to the headliner material). Went to my local fabric shop and bought 2-yards of black vinyl (there are a number of grades of vinyl---i bought the softest there was---about $8.00 a yard). Used the old headliner as a pattern and cut out the vinyl. Made sure it fit properly. Used a good spray-on contact cement to half of the roof and layed half the new vinyl headliner in. I let that dry for about 10 minutes then repeated the same process with the second half. Came out great----looks better than the old cloth headliner.
